Admission Process at Saint Laura Palliative and Hospice Care

Our admission process is designed to be swift, respectful, and easy to understand. We know that this time can be challenging for families, so we have made our steps simple and supportive to ensure everyone feels informed and comfortable.

  1. Referral: The process begins with a referral from a healthcare provider. This can be done by a doctor, hospital, or even by family members who believe hospice care might be beneficial. Our intake team is available to guide families and healthcare providers through this step.
  2. Initial Contact: Once we receive the referral, our admissions coordinator will reach out to the family. During this conversation, we will gather basic information about the patient, answer any immediate questions, and explain what to expect next.
  3. Evaluation: After the initial contact, we conduct a thorough evaluation of the patient’s condition and needs. This is done either in-person or through a virtual visit. The evaluation helps us understand the patient’s medical status, comfort needs, and preferences, which allows us to create a personalized care plan.
  4. Personalized Care Plan: Based on the evaluation, we design a care plan that is specifically tailored to meet the patient’s needs. This plan includes medical care, emotional support, and practical assistance for the patient and their family. We involve the family in this planning to make sure everyone is comfortable and that the patient’s wishes are respected.
  5. Admission and Transition: Once the care plan is developed, our team will work with the family to set up everything needed for the patient’s admission. This includes coordinating with doctors, arranging medical equipment, and ensuring all necessary paperwork is completed smoothly. We aim to make the transition to our care as gentle and stress-free as possible.
  6. Ongoing Support: After admission, our team remains by your side. We provide continuous support, ensuring the patient and family are cared for at every stage. We are here to answer questions, provide resources, and adjust the care plan as needs change. We also maintain close communication with the patient’s doctors to make sure all aspects of care are well-coordinated.
